Ticket prices are subject to change for tournaments including Districts, Regionals, and State Competitions. Booster passes will not be accepted at any FHSAA preseason, state series, or playoff events. Kids under 5 are free.

Athletic Forms & Physicals
For your child to participate in Florida High School Athletic Association (FHSAA) sponsored activities, a profile must be completed on Athletic Clearance and all necessary forms submitted electronically 3 business days prior to the first date of participation.
Please print and complete the Sports Medical Physical Examination form below as required by the (FHSAA). These forms can also be found on the Athletic Clearance website. This form must be submitted electronically through Athletic Clearance (via the Athletic Clearance Button below.) There is also a helpful instructional video on the Athletic Clearance website.
Sports Medicine
Providence School athletics is in a partnership with PT Solutions along with Southeast Orthopedic Specialists as our premier sports medicine providers. Through this partnership, athletes will have access to Athletic Trainer, Dylan Wiedeman from PT Solutions Physical Therapy and premier Team Physician, Dr. Kevin Murphy from Southeast Orthopedic Specialists.
PT Solutions offers complimentary injury consults to all Providence athletes, parents, staff members and friends at their Ponte Vedra or Bartram Oaks locations.
